Registration is
located inside the
front entrance. You or
your immediate
family members will
be asked to provide
demographic,
insurance, and other
information
concerning your
hospitalization.
TIP:
In order to speed up
your admission, have
your insurance care
and physician orders
ready to present to
the registration
clerk.

Consent |
You will be asked to
sign an
Authorization
Signature Form which
is a General
Conditions of
Admission, Consent,
Assignment of
Benefits, and
Financial Agreement.

Identification |
An identification
bracelet will be
prepared for you to
wear on your wrist.
The primary purpose
of the
identification
bracelet is for your
safety, so please do
not remove the
bracelet until you
leave the hospital.

Advance Directive |
Federal regulations
require hospital
personnel to inquire
as to the presence
of an Advance
Directive. This
includes a living
will or durable
power of attorney
with a healthier
clause. Information
on Advance
Directives is
provided to every
patient upon
admission. |
