Registration: |
Registration is
located inside the
front entrance. Your or
your immediate
family member will
be asked to provide
demographic,
insurance, and other
information
concerning your
procedure.

Pre-Certification: |
Depending on the
procedure you are
having performed,
some insurance
companies require a
pre-certification or
prior authorization.
This
pre-certification is
obtained by the
referring doctor's
office. As part of
the insurance plan
requirements, your
insurance company
may not pay for the
procedure if a
pre-certification or
prior authorization
is required for the
procedure but not
obtained before the
procedure.
TIP:
Verify with your
doctor's office that
the
pre-certification or
prior authorization
has been obtained,
if required.

Consent: |
You will be asked to
sign a consent form
for medical
treatment.

Identification: |
An identification
bracelet will be
prepared for you to
wear on your wrist.
The primary purpose
of the
identification
bracelet is for your
safety.
