Registration is located inside the front entrance. You or your immediate family members will be asked to provide demographic, insurance, and other information concerning your hospitalization.
TIP: In order to speed up your admission, have your insurance card and physician orders ready to present to the registration clerk.
You will be asked to sign an Authorization Signature Form which is a General Conditions of Admission, Consent, Assignment of Benefits, and Financial Agreement.
An identification bracelet will be prepared for you to wear on your wrist. The primary purpose of the identification bracelet is for your safety, so please do not remove the bracelet until you leave the hospital.
