Registration is located inside the front entrance. You or your immediate family member will be asked to provide demographic, insurance, and other information concerning your procedure.
Depending on the procedure you are having performed, some insurance companies require a pre-certification or prior authorization.
This pre-certification is obtained by the referring doctor's office. As part of the insurance plan requirements,
your insurance company may not pay for the procedure if a pre-certification or prior authorization is required for the procedure but not obtained before the procedure.
TIP: Verify with your doctor's office that the pre-certification or prior authorization has been obtained, if required.
You will be asked to sign a consent form for medical treatment.
An identification bracelet will be prepared for you to wear on your wrist. The primary purpose of the identification bracelet is for your safety.
